What You Need To Start Therapy:
- Have Pediatrician Referral faxed to 470-219-5336 that reads “Evaluate and Treat Speech Therapy” with your child diagnostic codes
- New Patient Intake Packet with HIPAA Acknowledgement and Attendance Policy
- Online Therapy Consent Form
- Current Individualized Education Plan/Program (IEP) or Individual Family Service Plan (IFSP) (if available)
- Specialist reports (if applicable) from: Neurologists, Developmental Pediatricians, Psychologists, Ear Nose and Throat (ENT) doctors, etc.
- Hearing Screening/Evaluation with results in Hertz required for PeachState or Amerigroup
